ASMR offers a variety of
case
managment services for clients living in southwestern New Hampshire.
Our services are comprehensive and include:
- Access
to the AIDS Drug Assistance Program (ADAP) in New Hampshire, referred
to as the CARE program
- Access
to the Housing Opportunities for People with AIDS
program (HOPWA), which provides assistance with short and long term
rental assistance, utility assistance and supportive funds for HIV/AIDS
related needs
- Assistance
related to medical aspects of HIV/AIDS
including cooperation with primary care and infectious disease doctors
as well as education related to drug adherence
- Educating
clients and their families about safer sex
practices to help minimize further transmission of the virus or other
sexually transmitted infections
- Facilitation
of contacts with local social services
agencies such as the Social Security Administration and the Department
of Health and Human Services in order to obtain and maintain benefits
which may include SSI, food stamps, Medicaid and Medicare
- Referral,
when appropriate, to other case management or
therapeutic organizations for issues related to finances, mental
health, substance abuse and recovery (e.g. budgeting, legal assistance,
concerns regarding death and dying and 12-step programs)
